| Company-Agent Agreement top
Between the Company “ Jerusalem prayer” and the Agent
The present Agreement is signed between the Jerusalem Prayer company
And______________________________________________________________
that
1._________________ is a Company Agent and he is awarded by an identification number_____________________.
2.___________________ is as a full- power representative of the Company to ________________ ( name of the region) to the term of __________________ ( months)
3. The Agent does the following work for the Company service:
- health and ______ prayers
- masses
- special church services
In order to do it he must be well – guided in Company Web-Sites,t o know their content and navigation. The minimum amount of deals is 4 –per day or 100 per month. If within 2 months' period the Agent doesn't fulfill the amount of work, the Company has the right to cancel the agreement unilaterally.
4. The payment for the orders may be done by the following ways:
- via Company web-site ( Pay Pall, Visa, Master Card, American Card)
- Bank transfer to the account number___________________
- Western Union transfer
5. The Agent _______________ is obliged to
- Control the payment – to receive a copy of the payment document
- To accept the order from the customer – to fill out the order blank
- To transfer the order to the Company no later than within 7 days
6. The Company confirms the order in a written form within 7 days.
7. On receiving the above confirmation the agent presents this confirmation to the customer within seven days.
8. The Agent keeps the account of the deals done and presents them to the Company every month no later than on the 10- of the month following the accounting period.
9.The Company pays the Agent the interest of __________________% every month, no later than on the 20-th of the month following the accounting period.
10. The Agent has the right to hire people to assist him in his work. He pays his assistants a part of his interest received from the Company in accordance with p.9.
the Agent is obliged to inform the Company of hiring the assistant and to present his personal data to the main office of the company.
The present Agreement is valid 12 months with the option of the following prolongation.
The Agreement may be canceled by both parties.
